The following form is 2 steps… The first step is the vital statistics we need to collect about your loved one, or if you are just informant then about the person you are contacting us about. The second step is where you select your package and then determine if you wish to do any add-ons.
Once you are completed with your package selection, you click submit at the bottom of the form, and you will receive an email with the information you have completed, included a detailed itemized list of the packaged items you’ve selected and our team will also receive the same information.
We will not have collected any payment information, but will be in touch with you shortly to confirm the details and finalize and contractual information before proceeding with the first step with your loved one.
